All physicians seeking participating in the HMO and Point of Service (POS) networks are required to complete the Credentialing process prior to participation. Credentialing is not a requirement for participation in the PPO network.

BCBSIL HMO/POS Targeted Specialties
Please complete and submit the following documents:
- BCBSIL Credentialing Application Cover Document
- SOI Health Care Professional Credentialing and Business Data Gathering Form or a completed Coalition for Affordable Quality Healthcare (CAQH) Application
- BCBSIL Attestation signed & dated within 60 days of submission
- Verification of Hospital Privileges
- Other required documents
- Current State Medical License
- Current Federal DEA.
- Current Controlled Substance License.
HMO Medical Groups / Practitioners
Along with the credentialing application, submit the HMO practitioner’s written service agreement with the Medical Group. No Applications will be processed without this document.
