We announced a new mailing address in September 2023 for faster claims processing and responses. Starting Oct. 1, 2024, mail sent to our previous address will be forwarded through the postal service to the new address. This could result in claims processing and payment delays. To avoid delays, update your contact information for us if you haven’t done so and submit paper commercial claims to the new address:

Forms updated with the new address are available to download, including the Claim Review, Corrected Claim, Additional Information and Requests for Recommended Clinical Review forms.
