Equal Hope to Reach Out to Members of Blue Cross Community Health PlansSM and Providers

In 2025, we’re collaborating with Equal Hope in an effort to reduce disparities in breast cancer detection and treatment for our members of BCCHPSM who receive care at South Side Healthy Community Organization and Westside Wellness in Chicago. Equal Hope is an organization in the Chicago area that focuses on access to care. 

Equal Hope will reach out directly to members who may have missed recommended breast cancer screening or treatment. Some of your patients may receive calls from Equal Hope to help them receive the screening and care they need. Equal Hope patient and nurse navigators may also reach out to you or your staff to request confirmation of appointments or information to ensure proper identification of members.

What does Equal Hope do?

Equal Hope provides patient and nurse navigators who:

  • Call members to help schedule appointments, address barriers to access and ensure members receive the screening they need
  • Work with hospitals to provide support throughout the diagnostic follow-up and cancer treatment process, as necessary
  • Identify social determinants of health and connect patients to health care and social service resources
  • Work with providers to request current patient information, recent patient visits confirmations and dates of recent visits

Equal Hope is an independent company that has contracted with Blue Cross and Blue Shield of Illinois to provide Wellness, Provider-Focused Cost/Quality Measures for members with coverage through BCBSIL.
