We contract with physicians, facilities and other health care professionals to form our provider networks to deliver quality, accessible and cost-effective health care services to our members.

Five Steps to Join
Step 1 – Complete an application
Individual and medical groups and clinics: To apply to join our networks, fill out the online Provider Onboarding Form. To add new networks to an existing contract, complete the Provider Onboarding Form for Contract as Solo or Add New Group/Clinic, then select the new networks you wish to join.
Check the status of your application by using our Case Status Checker. Enter the case number you received in your confirmation email.
The onboarding form helps us confirm your eligibility for participation. If you don’t meet eligibility requirements, we’ll notify you by email.
Independent lab providers: To apply to join our networks, complete the Independent Lab Questionnaire and submit online with the Provider Onboarding Form. Also complete the Independent Lab Supplemental Form.
Hospital providers (including long-term acute care facilities): To apply to join our networks, send an email to Facility Contract Notices.
Ancillary providers: To apply to join our networks, send an email to Ancillary Facility Contracts.
Long term services and supports: To apply to join our networks, send an email to Professional Government Contracting. This includes homemakers, meal delivery services, adult day services, home modification and personal emergency response systems.
HMO network: To join a commercial HMO network, first contract with a participating HMO Medical Group or Independent Practice Association. View the Commercial HMO MG/IPA Listing for details. Our commercial HMO networks include HMO Illinois®, Blue Advantage HMOSM, BlueCare DirectSM and Blue Precision HMOSM.
Out-of-state providers: Contact your local Blue Cross and Blue Shield Plan for information on network participation.
Step 2 – Submit a signed contract
Once we confirm that you meet eligibility requirements, we’ll email you a contract for the provider networks for which you qualify. Complete, sign and submit the contract according to instructions in the email.
Step 3 – Become credentialed, if required
The following provider types must complete the credentialing process prior to acceptance, then be recredentialed every three years:
- Professional provider types: MD, DO, PsyD, PHD, AUD, BCBA, OD, DC, CNM, DPM, LCSW, LCPC, LMFT, PA, APN, APRN, ANP, CNP, CNS, LAC, DN and RD
- Institutional providers: Hospitals and ancillary providers, including skilled nursing facilities, home health, home infusion and durable medical equipment suppliers
- Independent labs
Our credentialing requirements are derived from and in compliance with the State of Illinois and the National Committee for Quality Assurance credentialing standards.
Step 4 – Wait for final notification of acceptance
After we receive your signed contract and you complete the credentialing process, if required, we'll let you know if you're accepted into our networks. If you're accepted, you'll receive a welcome letter with your network effective date.
