Coverage

Your benefits plan offers:

  • A wide network of doctors and hospitals
  • Access to care out of state and outside the U.S. by using our BlueCard® Program
  • The option to go to any doctor in the network without having to pick a primary care physician or get a referral

Your coverage does not pay for services received outside of our provider network, except in the case of emergency care.

24/7 Nurseline

Call 1-800-299-0274 and a registered nurse will provide you with answers to your health-related questions, 24 hours a day, 7 days a week. With Nurseline, you can also learn about more than 1,200 health topics over the phone, via an audio library system.

Preventive Care

Did you know that many preventive services are covered? That means that you pay no out-of-pocket costs. These covered preventive services may include:

  • Blood pressure, diabetes and cholesterol tests
  • Many cancer tests such as mammograms and colonoscopies
  • Vaccines

Hearing Aid Benefit

The preferred hearing aid provider is EPIC Hearing Health care. Hearing aids provided by non-EPIC Hearing Health care providers will no longer be covered. For more information call 1-844-228-2227.

Fertility Benefit

Blue Cross Blue Shield Illinois for Caterpillar is utilizing the Fertility Solutions Program which is designed to help eligible employees and their spouses who may be experiencing fertility-related issues. The Fertility Solutions Program provides education and counseling and acts as an access point to a national network of premier reproductive treatment centers.

The program is available to employees and spouses who are enrolled in the Employee Health, Life and Disability Benefit Program and meet the Fertility Benefit's eligibility requirements.

Pre-certification and Contact Information

  • To be eligible to receive covered services under the Fertility Benefit, you must enroll in the Fertility Solutions Program and pre-certify infertility services. To enroll, pre-certify infertility services, get information on in-network providers/facilities or access educational information, call the Fertility Solutions Program specialists at 1-866-774-4626 (8 a.m. – 5 p.m. CT).
  • Note: If you don’t complete the pre-certification process, the infertility services you receive won't be covered under the Fertility Benefit, even if your provider or facility is part of the Fertility Solutions Program network.
  • You may also contact Customer Service 1-844-228-2227 who can provide Fertility Benefit coverage details and facilitate contact with the Fertility Solutions Program specialists.

Medical Policies

Blue Cross and Blue Shield (BCBS) Medical Policies are used to make benefit coverage determinations. 

Your plan does not cover certain services listed in the Medical Policies, such as Bariatric Surgery and Sleep Tests. To find out what is covered under your plan, view your benefit booklet or call 1-844-228-2227.
