Blue Choice OptionsSM

Blue Choice Options plan offers you the benefits of a Participating Provider Organization and the flexibility to choose a doctor within a large network. Visit the City of Chicago website for information about Blue Choice Options plans and other employee benefits.

How Blue Choice Options Works

With Blue Choice Options the decision of which provider you want to use is up to you. This plan offers you three levels of coverage, called tiers, which determine your out of pocket costs.

When you use our Provider Finder® to search for a doctor or hospital in the Blue Choice Options network, your results will include providers from your top two tiers which are both in-network.

In-Network Tier 1 Choose a Tier 1 provider to pay the least out-of-pocket costs. Look for the Blue Choice OPT label near the provider's name in your search results.
Tier 2 Choose a Tier 2 provider and you will pay more out-of-pocket costs than if your provider was in Tier 1. You'll see the PPO label near the provider's name in your search results.
Out-of-Network Tier 3 Tier 3 providers are NOT contracted in our Blue Choice Options or PPO networks so they are considered out of network. If you can't find your doctor in the search results, most likely he or she is out-of-network. You can still go to that provider but you will pay the highest out-of-pocket costs.

Urgent and Emergency Care are the only exceptions to these tiers. You will pay the same out-of-pocket costs regardless of which provider you go to.

You can find an independently-contracted doctor or hospital within your plan's network using the Provider Finder. Or, if you have a specific doctor or facility in mind, check Provider Finder to see their tier.
