Care Coordination

 These services cover:

• Planned inpatient surgical procedures.
• Unplanned admissions to an acute inpatient hospital or nursing care facility. 

These services help you if you need a lower level of care. We can help you move from one level of care to another. This may be when you are told you can go home from a hospital or a skilled nursing facility.

Have you just had behavioral health treatment? Are you not sure about what care to get next?  

Blue Cross and Blue Shield of Illinois has partnered with H.E.L.P. to get you the care you need. You may hear from them if you just had behavioral care. This may have been an inpatient admission or emergency department visit. They can give you resources, tips, and aftercare support.

This program is for members that have very complex health illnesses. Some examples of these may be kidney disease, depression, substance abuse and more. If you qualify, a Care Coordinator will reach out to you. They can help you use your benefits to create health goals to reach. 

You can sign up for or opt-out of Complex Case Management. Call Member Services at 1-877-860-2837 (TTY/TDD: 711) to learn more.

This program helps pregnant moms better understand and manage their pregnancies. The goal is for them to deliver a healthy baby without problems. If you are pregnant or have delivered a baby in the last 84 days, you may be able to join the program. 

In this program, you may get: 

• Education on pregnancy, postpartum, and newborn care. 
• Incentives for going to prenatal visits and postpartum visits. 
• Help finding a doctor to improve care access issues.
• A breast pump and extra benefits (Dental, Vision, Transportation).

If you have high blood pressure, diabetes or asthma, you can join this program. It will support you based on what you need. A Care Coordinator will work with you make goals to improve your total health.
