Care Coordination

As a member, you may work with a Care Coordinator to help with your health care needs. This includes your Managed Long Term Supports and Services (MLTSS), and any other care needs you may have.

Care Coordination helps with:

  • Nursing home care and supportive living facilities.
  • Medicaid-covered behavioral health services.
  • Non-emergency medical transportation.
  • Planning in-person visits or phone calls with you.
  • Listening to your health concerns and questions.
  • Learning how your benefits can improve your health.
  • Getting care services to find health issues before they get worse.
  • Setting up your visits with doctors and other health care team members.
  • Explaining your doctor’s orders, your health condition(s), your medications and your treatments.
  • Guidance before planned admissions, such as surgery.
  • Guidance after you leave the hospital after you were admitted.
  • Referrals to community programs and aids that offer more education and support.

Care Coordination for Home and Community-Based Services helps you manage:

  • Personal care attendants
  • Homemaker services
  • Home delivered meals
  • Adult day care
  • Nursing care service
  • Home health aide

Care Coordination is not required (except for waiver participants). You may join or opt out at any time. To join or opt out, call Member Services at 1-877-860-2837 (TTY/TDD: 711).
