Employer application for group coverage.

2. Benefit Plan Selection form (BPS)
Choose medical, dental and life plans.

Collect required documentation on group size, MSP and COBRA status.

Go to Form Finder. Click on Small Group from the left menu.

Benefit Plan Selection form (BPS)
Change, add or renew medical, dental and life plans. A BPS isn’t needed if a group renews without changes.

Benefit Program Application (BPA)
Use a BPA to update eligibility requirements or change premium periods.

Small Group Billing Method form
Submit a Small Group Billing Method Form for groups that are changing their billing method and have no other product or administrative changes. Get more details in our Small Group Billing Preferences Guide and FAQ.

Group Enrollment Application/change Form
Enroll new members or update existing memberships and plan selections.
