Develop a Quote

Use the "Quote a Group" eSales tool in your Blue Access for Producers account. The quoting tool allows you to create quotes for our fully- and self-funded small group plans.

Review Regulatory Events

Annual regulatory forms, reporting and requirements

Get New Sales Forms

Go to Form Finder. Click on Small Group from the left menu. Look for the following. Work with your general agent or sales account executive on the documentation necessary.

Benefit Program Application (BPA)
Employer application for group coverage.

Employer Group Information (EGI)
Submit required documentation on group size, MSP and COBRA status.

Request For Proposal Form
Get the RFP Request Form and complete it.
