Is Blue Cross Community Health Plans Right for Me?

Blue Cross Community Health PlansSM (BCCHP) is offered to individuals and families who are covered by HealthChoice Illinois. HealthChoice Illinois is also known as Illinois Medicaid. It is provided by the Department of Healthcare and Family Services (HFS). First you apply for Medicaid, and then you can choose BCCHP as your health plan if found eligible. For more information about Illinois Medicaid, visit the HFS website.

How Do I Join?

Joining is easy. There are two ways for you to enroll in Illinois Medicaid. Call or go online. Illinois Client Enrollment Services will send you more information about choosing a health plan once you are approved for Medicaid.

Am I Eligible to Enroll?

    Want to find out if you should apply to HealthChoice Illinois? For a step-by-step process, go to the ABE website.

    If at least ONE of these describe you, then you may qualify for Illinois Medicaid:

    • Child’s Health Insurance Program
      You are a family or child and you qualify for Medicaid through Title XIX or Title XXI.
    • Eligible for the following programs:
      You are under the age of 21 and are eligible for either Supplemental Security Income (SSI), Division of Specialized Care for Children (DSCC) or because of a disability.
    • Affordable Care Act
      You are an adult who qualifies for Medicaid as defined by the Affordable Care Act (ACA). This means your monthly income is less than 138% of federal poverty level.
    • You qualify for Medicaid but not Medicare and are:
      • age 65 or older; or
      • at least 19 years old and have a disability

    Don’t qualify for one of the above? You may still be able to enroll. Find out if you qualify for Managed Long Term Supports and Services (MLTSS).

  • Health Benefits for Immigrant Adults (Ages 42-64)

    Effective 1/1/24, members in the Health Benefits for Immigrant Adults (HBIA) and Health Benefits for Immigrant Seniors (HBIS) program transitioned from fee-for-service to Medicaid managed care.

    Enrollment in a health coverage program with a Managed Care Organization, such as Health Benefits for Immigrant Seniors (HBIS) and Health Benefit for Immigrant Adults (HBIA), is not counted in the public charge test. Enrollment in these programs will not affect your immigration status and immigration application. For more information about public charge, visit the Protecting Immigrant Families Illinois website or Keep Your Benefits Illinois website. You can also call the Illinois Coalition for Immigrant and Refugee Rights (ICIRR)  Family Support Hotline in English/Spanish/Korean/Polish: 1-855-HELP-MY-FAMILY (1-855-435-7693).

    You may be eligible for this program if you meet all 4 requirements:

    1. You are at least 42 years old and not over age 64.

    2. You are one of the following:

    • A lawful permanent resident (LPR) for less than five years (also known as having a green card), or  
    • An undocumented immigrant (including individuals in a Temporary Protected Status)
       

    3. You are a resident of Illinois.

    4. Your 2022 annual household income is at or below $18,754 for one person; at or below $25,268 combined income for a household size of two. The income limit differs depending on the number of household members.

  • Health Benefits for Immigrant Seniors (Age 65+)

    Effective 1/1/24, members in the Health Benefits for Immigrant Adults (HBIA) and Health Benefits for Immigrant Seniors (HBIS) program transitioned from fee-for-service to Medicaid managed care.

    You may be eligible for this program if you meet all 4 requirements:

    1. You are 65 years old or older 

    2. You are an Illinois resident 

    3. You are one of the following:

    • A lawful permanent resident (LPR) for less than five years (also known as having a green card), or  
    • An undocumented immigrant (including individuals in a Temporary Protected Status)
       

    4. Have an annual 2022 annual income at or below $13,590 or a couple with annual income at or below $18,310*

    5. Have assets below $17,500.

Your plan includes copayments (copays). Copays are fixed amounts you pay for a covered health care service. You usually pay a copay when you get the service. The amount you can be charged will vary depending on the service and the provider. No co-payment or cost sharing can be charged for an emergency service needed to evaluate or stabilize an Emergency Medical Condition. An Emergency Medical Condition is a condition with symptoms that are severe and painful enough that a reasonable person would think they are life-threatening and need immediate medical care. Things like a severe asthma attack, symptoms of a heart attack, or a car accident with serious injuries, are just some examples.
