About Your Blue Cross Community Health Plans℠ (BCCHP) Plan

BCCHP wants to make sure you get the health care you need. That’s why we offer benefits and programs to help keep you and your family healthy. Your plan covers doctor visits, lab tests, mental health care treatment, prescription drugs and much more. BCCHP pays for all Medicaid covered services. From routine care to urgent care, family planning to home health, BCCHP is there for you. You get full benefits, without copays.

What Is Covered?

  • Alcohol or drug treatments
  • Care during a hospital stay
  • Outpatient behavioral health services
  • Mobile Crisis Response services

Chiropractic Services

Limited to spinal manipulation for subluxation of the spine.

Services include:

  • Routine Exams
  • Cleanings
  • Fillings
  • Root canals (limited for members over the age of 21)
  • Dentures (limited for members over the age of 21)
  • Extractions

Yes, under certain circumstances, a Prior Authorization is required for non-covered benefits.

Emergency and Urgent Care Services

Call your PCP for follow-up care within two days of your emergency, or as soon as you can. You are also required to call Member Services to let BCCHP know you received care.

  • Birth Control
  • Family Planning and Education
  • Pregnancy tests
  • Tests for sexually transmitted diseases (STDs)

Services that are not included:

  • Fertility treatments
  • Surgery to reverse sterilization

  • Home health aide services
  • Speech therapy
  • Physical therapy visits

Hospital Emergency Room Visits

  • All medically necessary lab services
  • Cancer tests
  • X-ray services

Yes, these services must be ordered by your PCP.

Non-emergency Transportation Services

Transportation is covered to:

  • A doctor's appointment
  • An appointment with another health care provider
  • BCCHP sponsored events

You do not need prior authorization for standard non-emergency transportation services.

Vision Care

$0

Services include:

  • One (1) eye exam every 12 months per member
  • Eyeglasses:
  • Every two (2) years for member age 21 and older
  • Replaced as needed for members under 21
  • Additional reimbursement toward a pair of upgraded eyeglass frames
  • Contact lenses when medically necessary

Yes, under certain circumstances

1 Services must be medically necessary. Some services need approval from your health plan before you get treated. This approval is called "prior authorization." Your doctor should know which services need approval and can help with the details.

2 Some limits apply to general dentistry.

What Is Not Covered?

Health care that is not covered includes:

  • Cosmetic surgery or treatment that are not medically necessary
  • Procedures that are still being tested or experimental
  • Care from a non-licensed provider
  • Drugs and supplies without a prescription
  • Getting care in an emergency room for health issues that are not emergencies
